Begin by gathering basic details about when and where the incident occurred, who was involved (including witnesses), and what happened. Document this information promptly, ideally within 24 hours. Take photographs of any physical evidence, secure any relevant digital communications, and record your observations of the scene. This initial documentation helps establish a foundation for your investigation.

Use neutral language throughout your report, avoiding assumptions or judgmental terms. Document multiple perspectives, including those that contradict each other. Follow a consistent interview protocol for all participants, asking similar questions. Include direct quotes when possible rather than paraphrasing. Clearly separate facts from opinions or conclusions, and document your methodology to demonstrate thoroughness.

Limit access to investigation documents to only those with a legitimate need to know. Store physical documents in locked cabinets and password-protect digital files. Use initials or codes instead of full names when discussing the case with others. Remind all participants about confidentiality expectations.
